Membership Guidelines
This plan is only valid at Innovative Smiles and cannot be used at any other office. Children listed on the plan must live in the same household or be full-time students under 18 years old. THIS PLAN IS NON-REFUNDABLE. No refunds or premium reimbursements will be issued if a participant chooses not to use the dental plan. The 12-month membership fee is due in full upon enrollment, and membership becomes effective on the date payment is received. Payment is due at the time services are rendered. Members are responsible for scheduling appointments within the 12-month membership period to maximize benefits. Unused benefits are non-refundable. Renewal payment is due at the beginning of the original enrollment month each year. Membership rates may be reviewed and adjusted annually with notice. Membership plans exclude panoramic and cone beam X-rays. Membership expires one year from the enrollment date.
